The Medicaid Support Liaison is a key member of our team responsible for helping individuals and families navigate the Medicaid application process and access the services they need. This role focuses on personcentered support ensuring timely and accurate eligibility determinations connecting individuals to communitybased resources and advocating for equitable access to care. The ideal candidate is compassionate organized and committed to empowering others through direct service and systemlevel problemsolving.
- Client Guidance & Application Support
- Assist individuals and families in completing Medicaid applications and submitting required documentation.
- Explain Medicaid eligibility criteria benefits and timelines.
- Provide ongoing followup to ensure applications are processed and eligibility is determined.
- Support clients with completing additional forms such as financial eligibility and disability determination paperwork.
- Provider & Community Coordination:
- Coordinate with Case Management Agencies (CMAs) and ensure clients are successfully connected for further assistance.
- Maintain strong relationships with healthcare providers and community partners to support client needs.
- Build and manage a network of reliable providers to facilitate comprehensive access to care.
- Advocacy & Systems Navigation:
- Submit and track escalations or grievances on behalf of clients when issues arise.
- Actively participate in stakeholder meetings and community forums to represent client needs.
- Identify systemic barriers and work with leadership to improve Medicaid access processes.
- Outreach & Program Improvement:
- Conduct outreach and education activities to raise awareness about Medicaid programs and benefits.
- Contribute to strategic planning and service innovations efforts to enhance program effectiveness and reach.
- Collaborate with colleagues to share best practices and strengthen organizational impact
Qualifications :
- Understanding of Medicaid regulations Longterm Services and Supports (LTSS) or disabilityrelated programs preferred but not required
- Experience working in case management healthcare navigation or other regulated service settings preferred but not required
- Interpersonal Skills:
- Strong interpersonal and cultural competency skills for working with diverse populations including individuals with disabilities and families with complex needs.
- Experience advocating for clients in institutional or public service settings preferred but not required
- Organizational & Communication Skills:
- Exceptional time management followthrough and documentation habits
- Ability to communicate clearly and effectively in both verbal and written formats
Additional Information :
