Sharing experiences from their own journey as a caregiver the Family Partner under the direction of the Family Partner Supervisor provides Family Support and Training (Caregiver PeertoPeer Support)a service that involves a structured onetoone strengthbased relationship between a Family Partner and a parent/caregiver. The purpose of this service is for resolving a youths emotional and behavioral challenges by improving the capacity of the parent /caregiver to parent the youth. Through a mentoring relationship a Family Partner works to improve the parents functioning to support the youth in the community or to assist the youth in returning to the community. Specific activities include education assistance in navigating the child serving systems (DCF education mental health juvenile justice etc.); fostering empowerment including linkages to peer/parent support and selfhelp groups; assistance in identifying formal and community resources (e.g. afterschool programs food assistance summer camps etc.) support coaching and training for the parent/caregiver. Caregiver Peer to Peer Support services seek to help achieve goals in the individual child/youths plan.

1. Assist in the creation and maintenance of a positive and collaborative working environment where all employees of CHL and enrolled youth and their families feel welcome supported and comfortable.

2. All activities described here will be delivered at the offices in the community or in the familys home depending on the preferences of the enrolled youth and family.

3. Works to build strong functional working relationships with enrolled youth and their families

4. Develop and implement outreach strategies when necessary to enable parents/caregivers of enrolled child/youth to participate fully in the Wraparound Process (if enrolled in ICC).

5. Assists the clinical hub provider (Care Coordinator In Home Therapist or Outpatient Provider) to develop with each youth and family a Care Planning Team composed of family members their natural supports and if applicable state agency representatives school personnel providers and other significant persons.

6. Serves as a mentor to parents/caregivers to improve their ability to effectively advocate for their youth and navigate the child/family serving human services system including the familys active participation in the Wraparound process itself.

7. Works closely with the clinical hub provider the family and the Care Planning Team to monitor the Care Plan and serves as an advocate/liaison for the family when appropriate to access new or different services.

8. Participates as appropriate in all meetings training evaluation/research supervision quality management and related activities as may be directed by the Family Partner Supervisor.

Required:

1. High School Diploma/GED/HiSet.

2. Satisfactory CORI background check.

3. Current valid USissued drivers license and a registered inspected and insured automobile for work related purposes including transporting clients in a personal vehicle or a company

Required:

1. Demonstrates a basic functional knowledge of and endorses the system of care values: Child Centered and Family Driven StrengthBased Culturally Responsive Collaborative and Integrated and Continuous Quality Improvement. 2. Demonstrates clear and consistent boundaries.

3. Minimum 1 years experience working with youth/families.

4. Experience as a caregiver of a youth with special needs.

5. Has a current valid USissued drivers license and a registered inspected and insured automobile for work related purposes.

2. Satisfactory CORI background check.
