Job Description

Salary

The salary range for this position is CAD $49.20/Hr. CAD $63.47/Hr.

Summary

Reporting to the Operations Leader Emergency Department (ED):

Demonstrate expertise and role model best practices in assessing and managing acutely ill seniors with complex needs in the Emergency Department
Provide patient and family education to support selfcare and promote healthy behaviours
Develop implement and evaluate approved patient education programs
Collaborate with the Geriatric Care Team and ED interdisciplinary teams to provide guidance on care strategies for acutely ill seniors
Act as a visible and accessible clinical resource and role model for staff
Provide education and support to enhance staff knowledge and confidence in geriatric care
Create and evaluate education programs and patient education materials based on clinical best practices

What You Bring

Education

  • Registration with the British Columbia College of Nurses and Midwives as a practicing RN registrant
  • Three years recent related clinical experience in emergency or gerontological nursing including recent acute care experience and one years leadership experience familiarity with relevant Home Health programs completion of post graduate specialty program(s) and or Canadian Certificate in Gerontology or an equivalent combination of education training and experience.

Skills and Abilities

  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
  • Ability to coach and counsel patients families and staff.
  • Comprehensive knowledge of evidencedbased nursing practice relating to gerontological patients.
  • Demonstrated skill assessing gerontological patients identifying foci of care and initiating appropriate care plan in consultation with the patient/family and team members.
  • Ability to identify learning needs and develop educational programs.
  • Demonstrated ability to role model evidence based gerontological practice.
  • Ability to provide and/or develop resource materials including educational materials.
  • Ability to demonstrate knowledge of adult education principles.
  • Ability to work as a member of an interdisciplinary team.
  • Ability to apply decision making and analytical skills.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to role model conflict resolution strategies.
  • Ability to prioritize and organize work independently.
  • Knowledge of research processes and methodology.
  • Physical ability to carry out the duties of the position.
  • Ability to operate related equipment including computers.

Your Day to Day:

Demonstrate expertise and role model specialized geriatric care skills for acutely ill seniors with complex needs
Provide direct nursing care including assessments medication administration and treatments
Complete comprehensive geriatric nursing assessments in the ED using validated tools and clinical judgment
Monitor patient condition identify lifethreatening changes and intervene appropriately
Teach patients and families how to manage care administer treatments and use medical equipment safely
Develop and update patient/family education materials such as brochures manuals and instruction sheets
Assess readiness to learn by evaluating physical emotional and cognitive factors through assessments and collaboration with the care team
Collaborate with ED and Geriatric Care Team to provide education and direction for managing seniors care
Provide rapid geriatric assessments to support timely treatment discharge planning or admission decisions
Coach and educate staff through role modeling individual and group training and acting as a clinical resource
Facilitate discharge planning by making referrals arranging community supports and liaising with external agencies
Ensure continuity of care by following up postdischarge and updating care plans in collaboration with the interdisciplinary team
Advocate for patients and support development of healthpromoting behaviours in the context of illness
Support quality improvement by identifying trends and making program recommendations to leadership
Contribute clinical expertise to the development of standards policies and educational content
Participate in research quality initiatives staff development and internal/external committees
Maintain and update treatment records patient documentation and service statistics
Reprioritize tasks as needed based on changing patient and department needs
Perform other related duties as assigned
