As Health Services and Records Coordinator at Lincoln Glen Manor () an inviting senior community offering four levels of care & extensive amenities across an appealing 6acre campus you ll provide excellent communication skills ability to work individually & as part of a team energetic quick to learn creative/enthusiastic supporting those who need it most.
HIGHLIGHTS
As the patient advocate help them navigate their medical journey explaining available options
Work with patients to set appointments
Help patients fill out/understand medical paperwork
Coordinate efforts between doctors for multiple avenues of care
Oversee/manage health programs in hospitals/similar establishments
Identify patient/client needs & coordinate services to meet them
Assist elderly clients with medical needs/lowincome families in accessing necessary services
Managing patients health records/history; gathering/filing patient information
File/organize/retrieve patient records (paper & electronic files)
Maintaining patient files/face sheets ensuring patient records are current/organized
Review records for policy compliance; conduct audits
Process discharge papers
REQUIREMENTS:
High School diploma/equivalent plus:
Experience with seniors working in facilities/dementia training and/or exposure
Flexibility to work NOC shifts when needed
