Job Description
- Identifies eligible candidates facilitate program orientations complete evidencebased risk/needs assessment
- Maintains a caseload builds transition plans for clients to include individual case planning composes and collets required documents
- Assists participants with completing various forms and obtaining documents for transition training education and employment
- Reviews documents for accuracy completeness and compliance
- Compiles data for reports as needed
- Assists participants in gaining and retaining employment and provides referrals as necessary
- Networks with community providers employers and partners to establish vital contacts for case planning training education and employment
- Tracks and evaluates participants program compliance and noncompliance by working with agency divisions partners vendors and reentry team
- Resolves participant requests regarding payment issues freetime special purpose passes and grievances
- Maintains and adheres to site policies procedures and regulations and ensures confidentiality of all data/information
- Other duties as assigned
Qualifications :
- High School Diploma or equivalent with two years of related experience preferred
- Excellent computer skills and knowledge of Microsoft Office Word Excel Power Point
- Skills in communicating with diverse populations
- Experience performing case management and transition plans preferred
- Ability to work independently and have well developed interpersonal skills
- Experience in criminal justice or related human service field preferred
- Ability to travel as needed at least 25%
