Sentara Health Plans is recruiting for a Housing Services Specialist Remote in Glen Allen/Richmond VA!
Status: FullTimepermanent position (40 hours)
Standard working hours: 8:30 am to 5 pm EST MF
Location: This position can work remotely in Glen Allen/Richmond VA. Travel approx 12x a week up to a max of 1 hour driving distance in Central VA but mainly in Richmond.
Job responsibilities:
Part of the Social Determinants of Health Team (comprised of Housing Services Specialists and Social Workers)
The Housing Services Specialist assists with housing searches for members who are housing insecure or experiencing homelessness.
Work involves providing information and assistance to help members find lowincome housing options and guidance to apply for subsidized housing.
Coordinates with other staff to assist members transitioning from nursing facilities to the community.
Property visits and community networking to maintain a thorough knowledge of housing and supportive resources.
Manages a caseload through coordination and timely communication with members staff and service providers.
Duties are performed both remotely and in the field throughout the Central region of Virginia.
Education:
HS High School Grad or Equivalent REQUIRED
Certification/Licensure:
None required
Experience:
1 3 years experience in community outreach or customer service REQUIRED.
1 3 years experience in assisting older adults the elderly and persons with disabilities to secure accessible affordable housing REQUIRED.
1 3 years experience working with low income housing providers to develop accessible and affordable housing for older adults elderly and persons with disabilities REQUIRED.
Knowledge of subsidized housing and landlord/tenant rights preferred
Sensitivity to cultural and socioeconomic characteristics of the Cardinal Care population
