Overview:

Sentara Health Plans is recruiting for a Housing Services Specialist Remote in Glen Allen/Richmond VA!

Status: FullTimepermanent position (40 hours)

Standard working hours: 8:30 am to 5 pm EST MF

Location: This position can work remotely in Glen Allen/Richmond VA. Travel approx 12x a week up to a max of 1 hour driving distance in Central VA but mainly in Richmond.

Job responsibilities:

  • Part of the Social Determinants of Health Team (comprised of Housing Services Specialists and Social Workers)

  • The Housing Services Specialist assists with housing searches for members who are housing insecure or experiencing homelessness.

  • Work involves providing information and assistance to help members find lowincome housing options and guidance to apply for subsidized housing.

  • Coordinates with other staff to assist members transitioning from nursing facilities to the community.

  • Property visits and community networking to maintain a thorough knowledge of housing and supportive resources.

  • Manages a caseload through coordination and timely communication with members staff and service providers.

  • Duties are performed both remotely and in the field throughout the Central region of Virginia.

Education:

  • HS High School Grad or Equivalent REQUIRED

Certification/Licensure:

  • None required

Experience:

  • 1 3 years experience in community outreach or customer service REQUIRED.

  • 1 3 years experience in assisting older adults the elderly and persons with disabilities to secure accessible affordable housing REQUIRED.

  • 1 3 years experience working with low income housing providers to develop accessible and affordable housing for older adults elderly and persons with disabilities REQUIRED.

  • Knowledge of subsidized housing and landlord/tenant rights preferred

  • Sensitivity to cultural and socioeconomic characteristics of the Cardinal Care population
