Southwestern Veterans Center is seeking a hardworking passionate and caring Social Worker to join our team. DESCRIPTION OF WORK
As a Social Worker 1 you will be responsible for providing professional social work services support and interventions for residents at Southwestern Veterans Center. In this role you will participate as a member of the interdisciplinary care plan team by completing psychosocial assessments. You will also implement interventions that will assist and support the residents individual needs and preferences customary routines concerns and choices. Your duties will include ensuring all charted documentation is accurate completing behavioral assessments and counseling residents and family members. Additionally you will assess residents readiness for discharge and work with residents and their families to create a discharge plan.

Work Schedule and Additional Information:
- Fulltime employment
- Work hours are 8:30 AM to 4:30 PM Monday Friday with a 30minute lunch. This may change based on operational needs.
- Telework:You will not have the option to telework in this position.

REQUIRED EXPERIENCE TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- A bachelors or masters degree in social work or social welfare.
- Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree.
Other Requirements:

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will requirea medical examination.
- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police and FBI clearance.
