Job Description
- Full time ongoing position (SCHADS Level 5)
- Day to day combination of outreach officebased and WFH

Jewish Care Victoria offers a rewarding opportunity for a skilled Case Manager to provide strengthsbased clientfocused support within our Social Work team.
A particular focus of this position is to provide outreach support to people whose housing is at risk including people living in Jewish Care Vic transitional housing public or other community housing private rental and those living with family. This role involves working alongside individuals and families facing complex challenges coordinating tailored support that fosters housing stability wellbeing and achieve longterm positive outcomes.
You will be a great fit if you have:
- Tertiary qualification in Social Work (or equivalent)
- Minimum 5 years experience working in the community services sector including housing/homelessness and community mental health
- Proven expertise in conducting holistic assessments and risk assessments creating safety plans case management and participating in information sharing in line with MARAM CISS & FVISS guidelines
- A strong understanding of systems including housing mental health addiction community aged care and family violence
- Demonstrated understanding of assertive outreach interventions and complex casework with people who may be resistant to engaging with services
- Cultural sensitivity and a valuesdriven traumainformed approach
Screening criteria:
- National Police check
- Working with Children check and NDIS Worker Screening check or ability to obtain
- Valid Victorian Drivers licence
- Right to Work in Australia
