Job Description
What to expect.
You will coordinate assessments community integration activities planning linkage and advocacy. This position requires general and specific knowledge of crisis stabilization mental health communitybased programming and entitlement programs.
Specific responsibilities include:
- Supporting patients and addressing concerns as they arise including resource coordination appointments transportation skills training entitlements and money management.
- Maintaining accurate and complete documentation related to client needs and services provided.
- Implementing appropriate crisis stabilization techniques as necessary.
- Providing direct service for medication monitoring including assessment education and assistance with filling pill boxes.
- Providing transportation to clients using personal or companyowned vehicles.
- Facilitating hospital visits to potential community placements and organizing activities to assess community readiness.
- Participating in program evaluation and continuous quality improvement

What we need from you.
- A high school diploma or GED bachelors degree preferred.
- Two years of experience.
- A valid drivers license and safe driving record.
- Access to a 4passenger vehicle during work hours
- Vehicle insurance that names you as the insured driver.
