About the Role
We are seeking a skilled and compassionate Clinical Care Partner to join our Support at Home team in Franklin. In this pivotal role you will provide clinical nursing to our clients in the community act as a Care Partner for clients with complex needs and support our care team through mentoring supervision and leadership.
This position is ideal for a Registered Nurse (Level 2) with strong community or aged care experience who is passionate about personcentred care reablement and enabling people to age in place.
Key Responsibilities
- Deliver inhome clinical care including wound care medication administration chronic disease support and palliative care
- Provide clinical assessments and tailored care plans for clients with complex needs
- Support and mentor Care Partners Personal Care Workers and other team members
- Coordinate services with internal teams and external providers to ensure integrated care
- Lead clinical training onboarding and contribute to quality improvement and incident management
- Promote independence dignity and wellness for clients across service zones
Selection Criteria
Essential:
- Current AHPRA registration as a Registered Nurse (Level 2)
- Minimum 3 years experience in community aged or home nursing
- Strong knowledge of the Aged Care Act 2024 Aged Care Quality Standards and Support at Home program
- Demonstrated experience in care planning mentoring and team leadership
- Clinical competence in wound care dementia care medication management and palliative care
- National Police Check (within 3 months) WWVP card and Driver Licence
Desirable:
- Postgraduate qualifications in community nursing gerontology or chronic disease
- Experience in clinical governance and continuous improvement
