Job Description
The purpose of the Neighborhood SHOPP Case Management Program is aimed at maintaining and assisting homebound older adults in living independently in the community and reducing social
isolation. Utilizing a strengthsbased approach Case Management objectives include identification of needs and capabilities through a comprehensive intake and assessment process determination of eligibility for DFTAfunded inhome and social adult day services homedelivered meals and other supportive services; monitoring advocacy linkage to community services and resources followup on referrals and service provision. The Case Manager reports directly to the Social Worker Team Leader.
The position requires a Baccalaureate Degree in Social Work from an accredited school and experience in a social service setting. Knowledge and experience with the older adult population and community based services are also preferred. Essential competencies knowledge and skills include advocacy brokerage care coordination communication problemsolving collaborative teamwork and a dedication to empowerment and strengthbased social work practice. The ability to work with a diverse population and Bilingual skills in Spanish is helpful. Computer literacy is mandatory and knowledge of STARS is also helpful. The Case Management program follows a hybrid schedule with mandatory monthly home visits in the South Bronx. Conducting home visits may require walking up and down stairs. All staff must be able to travel and report to the office 23x a week and attend inpersontraining meetings or events.
