Job Description
ATC Healthcare Metro Detroit is looking for experienced Nurse Practitioners for openings with Gentiva Palliative Care.
Facilitates palliative care for Gentiva Palliative Care patients ensuring a comprehensive individualized treatment plan while demonstrating excellent clinical skills and ethical relevant judgment. Provides diagnosis treatment consultation and followup under the direction of the Palliative Medical Director (if required under State law).

Job Type:FullTime Days Some OnCall Required
Travel: 1020% local travel for patient visits training and team meetings (mileage reimbursed).
What Youll Do:
Deliver inhome and communitybased palliative care including diagnosis treatment and followup in collaboration with our Palliative Medical Director.
Develop and manage individualized care plans based on comprehensive assessments and patient/family goals.
Coordinate services across the care teamincluding Social Workers and Spiritual Care Coordinatorsto address physical emotional and spiritual needs.
Prescribe medications and order diagnostic tests and equipment within scope of practice.
Participate in rotating call coverage and provide responsive highquality care.
What Youll Need:
Masters degree in Nursing (MSN) and current Nurse Practitioner licensure (Family or Adult NP preferred)
Minimum 2 years experience as an NP with working knowledge of palliative care or serious illness management
Current Part B Provider status DEA license(s) and collaborative agreement (if required by state law)
Excellent communication documentation and critical thinking skills
Reliable transportation valid drivers license and auto insurance
Preferred:
ACHPN Certification (Advanced Certified Hospice and Palliative Nurse Practitioner)
Experience with Part B billing and pointofcare software
EDUCATION
Graduate of an accredited school of nursing and an accredited nurse practitioner program usually at the masters degree level.
EXPERIENCE
At least one year experience as a nurse practitioner in a clinical setting.
CREDENTIALS:
CPR as required by client facility; health and screening tests as required by specific facilities and/or regulatory agencies.
