Job Description
ATN Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Health team. The LPN will provide quality nursing care to a home health client ensuring their health safety and wellbeing. The ideal candidate will have experience in home health care and a passion for improving the lives of patients.
Responsibilities:
- Patient Care: Provide direct nursing care to the client according to the care plan developed by the registered nurse and physician.
- Assessment: Conduct regular assessments of the clients health status and report any changes to the healthcare team.
- Medication Management: Administer medications as prescribed and monitor for side effects or adverse reactions.
- Wound Care: Perform wound care and dressings as needed.
- Dressing
- Transfer
- Mobility
- Vital Signs Monitoring: Monitor and record vital signs including blood pressure pulse respiration and temperature.
- Documentation: Maintain accurate and detailed records of patient care observations and treatments.
- Education: Educate the client and their family on healthcare practices selfcare techniques and disease management.
- Communication: Communicate effectively with the healthcare team client and family members to ensure coordinated and continuous care.
- Compliance: Adhere to all federal state and local regulations and guidelines as well as the policies and procedures of ATN Healthcare.
- Supervise CNAs Home Health Aids and care givers.
- Licensure: Current and valid LPN license in the state of Colorado.
- Education: Graduation from an accredited LPN program.
- Experience: Minimum of one year of experience in home health care preferred.
- Skills:
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficient in electronic health records (EHR) and documentation.
- Certifications: Current CPR certification.
- Transportation: Reliable transportation and valid drivers license.
- Ability to lift and move patients as required.
- Prolonged periods of standing walking and moving.
- Ability to work in various home environments.
- Flexible scheduling with the potential for weekends holidays and oncall duties as needed.
