Erie Family Health Centers a top workplace in Chicago and suburbs is looking for a valuable addition to our Care Management team! Care Managers (CM) partner with licensed care management staff members and other care team members to deliver a comprehensive highquality patientcentered experience. Care Managers conduct timesensitive outreach and/or provide support which includes short and longterm capacities to coordinate care for highrisk and 2 diseasemanagement patients

Main Duties & Responsibilities

  • Conducts outreach to complete between 90120 health risk assessments per month.
  • Conducts successful outreach to 120 disease management eligible or enrolled per month.
  • Collaborates with Eries Licensed Care Managers (LCMs) Transitions of Care (TOC)team the Social Drivers of Health (SDOH) Health Promotions and all other patient program team members to provide care coordination services and link with best fitprogram for patient needs.
  • Documents care team communication resulting action steps and activities in Eries electronic health record (Epic) for all participating patients aligned with Erie and CARES Act standards (e.g. no medical jargon etc.).
  • Documents all activities in additional external system as required/appropriate.

Qualifications

Education

Required:

  • High School diploma or equivalent required

Preferred:

  • Community Health Worker training
  • Bachelors degree

Experience

  • At least 1 year of experience in communitybased organizations safety net settings or with vulnerable population required
  • One year of customer service experience preferably in a health care setting preferred
  • Experience conducting patient assessments preferred.
  • Phonebased outreach experience preferred.
  • Experience identifying referring to and working with communitybased organizations preferred.
  • Experience with health system navigation and patient education preferred.
  • Managed care care management or care coordination experience or exposure preferred.

Skills and Knowledge

  • Strong computer skills required: expert in Microsoft Office Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system etc.).
  • Attentive to detail required
  • Strong interpersonal and oral communication skills required
  • Comfort using phonebased translation services required.
