Blanchard Valley Health System in Northwest Ohio is seeking a compassionate and experienced Nurse Practitioner with a strong commitment to palliative care hospice and geriatrics.

This position is for INPATIENT care.
Deliver care in a 75% inpatient / 25% outpatient setting
Provide palliative care services primarily at Lima Memorial Hospital
Work MondayFriday 8:00 AM5:00 PM managing approximately 6 patients per day
Practice autonomously to manage symptoms and chronic diseases
Lead goals of care discussions and provide advanced care planning
Round on inpatient hospice patients as part of daily duties
Collaborate with hospitalists who manage patient admissions
Function within an open ICU environment
Participate in a rotating oncall schedule
Role requires experience as an advanced provider in acute care palliative care or hospice
Ideal candidates will have a strong passion for palliative care hospice and geriatrics

Not accepting new graduate candidates experience required

Compensation and Benefits:
