About the Role:
In this position youll provide inhome primary care to patients across your region including:
Conducting comprehensive assessments and developing care plans
Managing chronic conditions and performing routine medical procedures
Collaborating with interdisciplinary teams to ensure highquality outcomes
Building meaningful relationships with patients and families
Travel Requirement:
This is a travelbased position that requires you to be in the Chico area on a weekly to biweekly basis. A travel stipend will be provided for visits to this region. When not in Chico youll serve patients in surrounding areas based on need and availability.
