About the Role
You will undertake pharmacy specific assessments plan and implement strategies to support appropriate and quality use of medicines for clients in the community.
As the single program Pharmacist you are required to receive referrals from care coordinators and manage a caseload of clients. The Pharmacist will work with and in support of care coordinators and other team members to assist the implementation of client goals. The pharmacist will ensure effective management of Pharmacy aids and equipment to maximise client independence and safety.
Secondary consult is an integral part of the role providing advice and support to team members on medications and their management.
Classifications for this position will range from classification range SX6 SX81 ($108820.40 $123905.60 per annum).

About Complex Care and Chronic Disease (CC/CD)
The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present or who are at risk of presenting to hospital because of complex medical and or psychosocial issues. Clinical service delivery is undertaken in the clients own home and their local community.

LOCAL WORK ENVIRONMENT
This position is part of the Health Independence Program (HIP) which is comprised of four streams: Pain Management Disability Services Complex Care/Chronic Disease and Access Rehab & Clinics. The intent of all HIP services is to deliver improved outcomes for clients and to support hospital demand management and flow by delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centered care planning.
This position will be part of the HIP Complex Care and Chronic Disease Service (CC/CD). The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present or who are at risk of presenting to hospital because of complex medical and or social issues. CC/CD currently consists of multidisciplinary teams based at SVHM Fitzroy and at North Richmond Community Health Hub. Clinical service delivery is undertaken in the clients own home and their local community.

POSITION PURPOSE
The Pharmacy Senior Clinician undertakes pharmacy specific assessments plans and implements strategies to support appropriate and quality use of medicines. Pharmacy intervention will support the health wellbeing and selfmanagement of any client accessing HIP services
As the single program Pharmacist you are required to receive referrals from care coordinators and manage a caseload of clients including prioritisation of those most at risk. The Pharmacists will work with and in support of care coordinators and other team members to assist the implementation of client goals. The pharmacist will ensure effective management of Pharmacy aids and equipment to maximise client independence and safety.
Secondary consult is an integral part of the role providing advice and support to team members on medications and their management. Active participation in the multidisciplinary case conference is a requirement of the role. The Pharmacy Senior Clinician responsibilities involve the coordination and liaison with SVHM Pharmacy department and relevant external Pharmacy bodies (e.g. SHPA and PSA) and supporting professional development activities. This includes liaison with the Senior Pharmacist Medication Safety and Strategy as required for any pharmacy specific matters.
