About the Role

You will undertake pharmacy specific assessments plan and implement strategies to support appropriate and quality use of medicines for clients in the community.

As the single program Pharmacist you are required to receive referrals from care coordinators and manage a caseload of clients. The Pharmacist will work with and in support of care coordinators and other team members to assist the implementation of client goals. The pharmacist will ensure effective management of Pharmacy aids and equipment to maximise client independence and safety.

Secondary consult is an integral part of the role providing advice and support to team members on medications and their management.

  • Undertake Pharmacy specific assessments and plan and implement appropriate Pharmacy interventions and goals
  • Provide secondary consultation in regard to medications and their management
  • Ensure effective management of Pharmacy aids and equipment to maximise client independence and safety.
  • Provide Pharmacy clinical expertise and leadership in clinical/team meetings.
  • Participate in Pharmacy specific supervision.

Classifications for this position will range from classification range SX6 SX81 ($108820.40 $123905.60 per annum).

Your Contribution

  • Bachelor of Pharmacy or higher degree.
  • Current AHPRA registration needs to meet CPD requirements of pharmacist who conducts medication reviews
  • Registered (unrestricted) with the Pharmacy Board of Australia
  • Victorian Drivers License
  • Postgraduate qualification in Hospital/Clinical Pharmacy from a recognised institution or society e.g. Certification in Geriatric Pharmacy (SHPA) or Credentialed to conduct medication reviews (Australian Pharmacy Council.
  • Knowledge of St Vincents Patient Management and Information Technology systems and databases would be an advantage
  • Current National criminal history check or willing to obtain

The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present or who are at risk of presenting to hospital because of complex medical and or psychosocial issues. Clinical service delivery is undertaken in the clients own home and their local community.

LOCAL WORK ENVIRONMENT

This position is part of the Health Independence Program (HIP) which is comprised of four streams: Pain Management Disability Services Complex Care/Chronic Disease and Access Rehab & Clinics. The intent of all HIP services is to deliver improved outcomes for clients and to support hospital demand management and flow by delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centered care planning.

This position will be part of the HIP Complex Care and Chronic Disease Service (CC/CD). The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present or who are at risk of presenting to hospital because of complex medical and or social issues. CC/CD currently consists of multidisciplinary teams based at SVHM Fitzroy and at North Richmond Community Health Hub. Clinical service delivery is undertaken in the clients own home and their local community.

The role is accountable to

  • HIP Complex Care and Chronic Disease Manager for all operational responsibilities.

  • The Manager of the Pharmacy Department SVHM for clinical governance.

POSITION PURPOSE

The Pharmacy Senior Clinician undertakes pharmacy specific assessments plans and implements strategies to support appropriate and quality use of medicines. Pharmacy intervention will support the health wellbeing and selfmanagement of any client accessing HIP services

As the single program Pharmacist you are required to receive referrals from care coordinators and manage a caseload of clients including prioritisation of those most at risk. The Pharmacists will work with and in support of care coordinators and other team members to assist the implementation of client goals. The pharmacist will ensure effective management of Pharmacy aids and equipment to maximise client independence and safety.

Secondary consult is an integral part of the role providing advice and support to team members on medications and their management. Active participation in the multidisciplinary case conference is a requirement of the role. The Pharmacy Senior Clinician responsibilities involve the coordination and liaison with SVHM Pharmacy department and relevant external Pharmacy bodies (e.g. SHPA and PSA) and supporting professional development activities. This includes liaison with the Senior Pharmacist Medication Safety and Strategy as required for any pharmacy specific matters.

5. POSITION DUTIES

  • Undertakes Pharmacy specific assessments and plans and implements appropriate Pharmacy interventions and goals using best practice principles and evidence based practice to achieve Quality Use of Medicines (current national guideline).
  • Provides secondary consultation in regard to medications and their management across Complex Care Service and the ALERT Program.
  • Ensures regular liaison communication and consultation with the SVHM Pharmacy department to support integration and maintenance of professional standards and legislative requirements.
  • Ensures regular review of Pharmacy Service caseload and client prioritisation.
  • Ensures effective management of Pharmacy aids and equipment to maximise client independence and safety and minimise organisational risk.
  • Actively participates in the evaluation of the effectiveness of the CCS Pharmacy Service and/or identifies supports or leads Pharmacy specific quality improvement activities.
  • Provides Pharmacy clinical expertise and leadership in clinical/team meetings.
  • Participate in Pharmacy specific supervision.
  • Communicate effectively and professionally with a wide range of team members colleagues key stakeholders and external service providers.
  • Assessment and management of medication related risk issues including timely documentation of the plan and escalation.
  • Perform within HIP guidelines and specific service guidelines ensuring that care is provided in accordance with the SVHM HIP model of care.
  • Ensure all documentation client records and activity recording are completed and managed in a timely manner consistent with SVHM and HIP policies and procedures legislative and regulatory requirements.
  • Participate in quality initiatives team processes discipline specific service improvements and service development activities.
  • Demonstrate accountability to HIP through professional communication active participation information sharing evaluation statistical / data collection and analysis.
  • Meet Activity Based Funding target requirements.

6. INCUMBENT OBLIGATIONS

  • Minimum of seven (7) years postgraduate experience
  • Postgraduate qualification in Hospital/Clinical Pharmacy from a recognised institution or society e.g. Certification in Geriatric Pharmacy (SHPA) or Credentialed to conduct medication reviews (Australian Pharmacy Council.
  • Demonstrates clinical expertise through knowledge of research new developments and evidencebased practice in discipline specific and related areas.
  • Highly developed interpersonal skills that incorporate the qualities of excellent communication and collaboration
  • Highly developed skills of organisation planning and problem solving
  • Ability to identify and build relationships with key stakeholders to the position
