Summary of Duties
As a Nursing Facility Client Care Surveyor you will:
Conduct survey work is to survey the quality and adequacy of care and services received by residents of Nursing Facilities (NF) for substantial compliance within mandated federal regulations and state rules within required time frames.
Work with a team of surveyors either as a team member or a team coordinator working both independently for some tasks and in a consensus process for others.
Write reports of findings using specified CMS computer software program and according to CMS Principles of Documentation guidelines. Reports must be clearly descriptive of findings concise complete and legally defensible.
Conduct intake/triage work to capture review clarify and augment through interview and record review all relevant data received for NF public complaints and facility reported incidents using the federal database.
Monitor all incoming NF complaints and incident reports.
Create new intakes and enters all complaint/incident intake information into the CMS database.
Contact complainant(s) to verify and confirm information received and solicit additional information to assist in determining potential noncompliance with federal and/or state requirements.
Minimum Qualifications
A valid drivers license and acceptable driving record are required for this position.
Three years of experience in areas related to nursing social work developmental disabilities dietetics/nutrition or sanitation AND one of the following:
A Bachelors degree in a health or human services field
OR
A license as a Registered Nurse.
LPN certification and/or experience will not meet the minimum qualifications.
Essential Attributes
We are looking for candidates with:
Experience in areas related to Nursing (RNs encouraged to apply) Social Work Speech Pathology Physical Therapy Occupational Therapy Investigations or Nursing Home administration.
Skill in critical thinking exercising independent judgment and decisionmaking in the application of laws rules regulations policies or procedures.
Experience writing legal or medical reports.
Experience communicating verbally with medical professionals residents and others sometimes under tense or hostile conditions.
Experience and knowledge of federal and state laws rules and regulations related to the provision of care and services in health or healthrelated settings.
Team player: Display a willingness to help others remain calm in tense situations and respectfully communicate with coworkers.
Experience and proficiency typing taking notes and writing reports using computers/laptops and using programs including Teams Word Outlook and SharePoint.

Working Conditions
Hybrid remote/onsite work is required for this position. The first month or more you will need to be in the Tigard office to complete required training.
Daily or weekly you will be traveling to facilities or offices to complete investigations or attend inperson meetings trainings or activities.
You will primarily work within a variety of statewide longterm care facilities and working conditions (such as small conference rooms standing for long periods of time working on a laptop computer etc.).
Lift and carry up to 20 pounds throughout each day.
Irregular work schedules and frequent schedule changes including occasionally working long days nights overnight travel and weekends/holidays.
You may interact with a wide range of people including some who have experienced trauma and exhibit challenges managing their emotions.
You will be a mandatory reporter of abuse and neglect of children older adults and other adults with disabilities.
Extensive daily use of electronic devices (computer wireless communication devices etc.).
You will be required to frequently travel and drive statewide at times in inclement weather long distances and overnight for variable lengths of time to and from NFs offices for meetings training and surveys.
State and rental cars are available for use as well as some mileage accommodations and meal reimbursements are provided.
This position is not eligible for overtime. However straight time is awarded for use in addition to other paid time off.
Typical work hours are 8am5pm Monday through Friday: at times required to work more than scheduled hours on weekends/holidays or in the event of serious or emergent situations.

The Oregon Department of Human Services does not offer visa sponsorship.
