The RN uses the BSWH nursing model to coordinate patient care. Using the nursing process the RN assesses the patient and identifies nursing diagnoses. The RN develops and implements an individualized care plan and evaluates the patients response. The RN promotes safe passage for patients by using knowledge of patient needs. The RN helps patients transition through healthcare without preventable complications or delays.
