The Juvenile Justice Services Program consists of case management restorative justice conferences and clinical services for up to 40 youth comprised of PINS Juvenile Offender/Juvenile Delinquents JDRaise the Age and Adolescent Offenders as referred by Nassau County Probation Department. The program design offers family focused treatment services that have been shown to be effective in addressing problems associated with risk of delinquency.
The Case Planners overall focus will be to improve individual functioning and family dynamics to prevent out of home placement for the youth. The Case Planner responsibilities include maintaining regular contact with the youth and their families at least once per week and implementing the various parts of the program model. In addition they will provide case management services life coaching parent skill training and make all collateral contacts as necessary. The Case Planner for the Nassau County Probation Juvenile Justice Services at Hope For Youth reports to the Program Supervisor.
To be a successful Case Planner you must be ready willing and able to do the following always:
- Function within Agencys policies and procedures as outlined in the Employee Manual.
- Function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
- Provide support to the clients and familiesi.e. the knowledge skills and tools necessary in order for them to maintain the child in their home.
- Achieve successful outcomes within the guidelines outlined in NYS County and HFY policy and procedure (shared).
- Perform other resulting and necessary work to meet the needs and requirements of the clients the program Hope For Youth oversight agency and any other regulatory obligations
How you will generally spend your time in this job:
- Maintaining regular contact with the youth and their families at least once per week and implementing the various parts of the program model (including evenings nights and weekends) for caseload (up to 20 cases) throughout Nassau County utilizing your own vehicle.
- Providing case management services life coaching parent skill training and make all collateral contacts as necessary.
- Regularly reviewing the service plan with the youth and family.
- Completing the initial CANS 2.0 and all subsequent CANS 2.0 as well as attending school meetings and court appearances with the families as necessary and be responsible for discharge/transition planning.
- Documentation and related paperwork
- Supervision staff meetings treatment team meetings and conferences
- Training and staff development
- Other related assignments
To qualify for this job you need to be or have at minimum:
- BSW (or Bachelors in a related filed) plus 2 years related casework experience or MSW
- Excellent written verbal and computer skills
- Flexible schedule to accommodate clients and program
- Bilingual Spanish/English speaking preferred
- Access to a reliable vehicle that can be used on a regular basis for home visits
- Physical Requirements: read/write reports and log entries walking/standing for prolonged periods bending/stooping kneeling/squatting climbing stairs reaching overhead repetitive wrist/finger movements driving own vehicle
