Job Description

Under the direction/supervision of the clinical team you will perform direct patient services tasks.

Accountabilities:

  • Assisting with patient activities of daily living mobilization and transport
  • Encourage and support patients in their own care and rehabilitation to foster a sense of autonomy. 
  • Observe and document changes in patient condition and mental status; they will report any concerns to the nursing team.
  • Communicate effectively with patients families and other healthcare professionals ensuring clear an accurate information sharing.
  • Follow care plans delegated to them by nursing team within their scope of practice.
  • Maintain a safe environment they are involved in ensuring a space that adheres with universal falls precautions.
  • Communicate need to clerical or resource staff to order supplies
  • Responsible for daily restocking of supplies in patient care rooms and supply storage areas
  • Maintain and distribute departmental monitoring equipment
  • Contributing to the maintenance of a safe work environment

 


Qualifications :

  • Registered with Health and Supportive Care Providers Oversight Authority (HSCPOA)
  • Personal support worker certificate required from an accredited institution
  • Basic knowledge of aseptic techniques
  • Current CPR
  • Recent related experience working with patients in a rehabilitation setting
  • Recent experience working in a highly technical patient care environment
  • Excellent organizational and prioritization skills
  • Excellent interpersonal communication and comprehension skills
  • Flexibility to meet unit needs
  • Ability to work well with team members
  • Basic computer skills and knowledge of Meditech system
  • Must demonstrate Oslers Values of Respect Excellence Service Compassion Innovation and Collaboration
  • Excellent attendance punctuality and work record
  • Ability to travel between and work at all Osler sites as required
  • Able to handle physical requirements of the job (ability to lift at least 2530 pounds)


Additional Information :

Currently Days/Evenings 8 hours including weekends and statutory holidays
(subject to change in accordance with operational requirements)

Salary:
Minimum:  $30.76 per hour
Maximum: $31.38 per hour
