POSITION DESCRIPTION:Service Coordinator

RESPONSIBLE TO: Director of Program Services

POSITIONS SUPERVISED: None

DESCRIPTION: This is a professional level position which requirescoordinating services for adults who have an intellectual ordevelopmental disability. This includes but is not limitedto coordinating services to meet medical psychiatricsocial nutritional educational vocational employmenthousing transportation recreational legal and advocacyneeds. Service Coordinators provide support to individualsto actively engage in and direct their services.Service Coordinators assist in designing the supports thatwill be most effective in achieving goals. ServiceCoordinators advocate for people receiving supports andassist with decision making about the supports and servicesthey want and need.

QUALIFICATIONS: Detail Oriented. Ability to multitask. Strong oral andwritten communication skills. Able to work as a teammember. Qualified candidates will possess aBachelorsDegree or equivalent in human services social worksociology or psychology and2 years ofexperienceworking in a similar experiencemay be substituted for education.

INTERRELATIONSHIPS:Work closely with Case Managers. Work closely withagency staff.

RESPONSIBILITIES:

  • Coordinate and monitor Individual Support Plans (ISP).

  • Complete assessments with people supported and/or coordinate Direct Support Professionals to complete assessments as requested by the Conflict Free Case Manager (CFCM).
  • Participate in ISP meetings to assist in development of a comprehensive personcentered plan.

  • Write ISP Programs.

  • Correspond with assessment results and the Outcomes/Action plans of the ISP.

  • Assist with medical appointments as needed.

  • Identify rights restrictions and coordinate due process for all rights restrictions.

  • Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate.
  • Review all incident reports (medical behavioral critical) and ensure appropriate follow up.

  • Ensure family guardians CFCMs and other advocates are notified regarding change in condition behavior treatment or service plans.

  • Complete required documentation of services including input of data reports and plans into Therap.
  • Gather documentation and remediate SMART Reviews.
  • Ensure all necessary federal state and agency forms are completed (ex. EA240; Housing; Social Security; Therap updates (IDF) etc.).
  • ICAP Preparation/Completion as needed.
  • NCI CQL ARSD Survey/Preparation as needed.
  • Participate in Abuse/Neglect Investigations as requested.
  • Other duties as assigned by supervisor.

REQUIREMENTS:

The following actions are performed frequently:

Lifting >10 lbs. see sit stand reach handle finger (pinchtype activity) repetitive movement talk and hear.

The following actions are performed occasionally:

Kneel crouch walk lift>25 lbs. push pull bend carry and balance.

The following actions are performed seldom:

Lifting>50 lbs.
