Job Description

$26.00 $28.00/Hour

RESPONSIBLE TO: Adult Day Health Care Director

GENERAL

DESCRIPTION: Works closely with the RN in all aspects of registrant care to insure continuity of registrant care.

Responsible for the delivery of care as directed by the RN.

Administers medications and charting of it in the appropriate places.

Performs treatments etc. and charting of it in the appropriate places.

Supervises and instructs CNAs in the performance of their duties.

FUNCTIONS: Must be available to periodically work more than eight (8) hours per day or more than forty (40) hours per week.

Administer medications and treatments in accordance with written Physicians orders and established policy and procedures and record it in appropriate places.

Report to RN and ADHC Director any document changes in physical and emotional status unusual and/or abnormal changes.

Check medication/treatment supplies and reorder as necessary

Carry out ADHC administration and registrant care duties as directed.

Supports and demonstrates practice consistent with philosophy standards policies and procedures of St. Luke ADHC.

DUTIES AND RESPONSIBILITIES:

Monitor ADHC staff for compliance of established policies and procedures.

Assist in emergency care as necessary.

Give concise accurate qualitative report to RN.

Receive report from RN.

Transcribe physicians orders and complete all necessary forms (i.e.: lab requests xray requests).

Assist with orientation instruction and supervision of other registrant care personnel.

Assist the RN with evaluation of registrants needs condition and care.

Review registrants medical and social history.

Relieve CNAs for breaks and mealtime when necessary.

Monitor registrants mealtimes as directed.

Direct and assist with unit cleaning as well as the care of supplies equipment and appliances.

Follow proper policy and procedure with maintenance of controlled substances (i.e.: administration count documentation of effectiveness).

Clean and maintain the medication room. Also clean tops and sides of bottles that contain liquid medications.

Assist Activities Staff when available for all activity functions.

Super Superviseand assistCNAs in all care areas assuring use of proper procedure to insure that registrants are receiving adequate assistance with personal hygiene and daily activities.

Give report to CNAs giving them instructions on special concerns (ie: V.S. condition of wounds catheter care issues etc.)

Receive reports from CNAs at the end of the shift and as concerns are addressed.

Handle any difficulties and/or problems in the ADHC. Consult with the RN or Director on those problems where assistance is needed.

Insure that weights and vital signs are taken and documented as ordered (i.e.: monthly weekly daily).

Verifies information on Consultation Sheet prior to completing the forms and see that registrants are ready for all scheduled appointments on time. Order early meal trays as indicated.

Assist the RN or Director with all necessary paperwork and transportation arrangements for emergency care for registrant. Check to see that transfer form is complete and accurate.

Report accidents/incidents or medication errors to RN or Director and complete appropriate forms.

Maintain CONFIDENTIALITY of all registrant care information.

Participate in St. Lukes Quality Assurance Program.

Assist the CNAs with registrant care as needed.

Other related duties that may become necessary or as directed by a supervisor department head and/or administration.

WOR WORKING CONDITIONS:

Works in welllighted/ventilated areas.

Walking sitting standing bending lifting and moving intermittently during working hours.

Subject to frequent interruptions.

Exposed to infections and odors.

Involved with registrants family members personnel visitors etc. under all conditions/circumstances.

Communicate with nursing staff and supervisors.

Willingness to work beyond normal hours.

Attend and participate in continuing educational programs as directed and required.

QUALIFICATIONS: Graduate of an approved School of Nursing.

Current LPN. licensed in the State of New York.

Experience in Charge position preferred.

Previous experience in geriatrics helpful but not necessary.

Understands the special needs of geriatric and MR/DD registrants.

A willingness to learn through inservice and continuing education Programs.

Must be able to supervise and instruct others professionally.

Must be wellgroomed courteous tactful patient kind and pleasant when dealing with registrants visitors and coworkers.

Must have a good sense of responsibility.

Ability to remain calm in emergency situations.

CPR certification required.
