Job Description

The BedfordStuyvesant Family Health Center (BSFHC) is a patientcentered team based multispecialty practice in central Brooklyn. BSFHC has been providing affordable comprehensive outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC) BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. Our mission is to provide the most professional courteous and highest quality health care with dignity to those we serve especially the undeserved population without regard for ability to pay.

We are seeking an Outreach Health Educator to join our Access Health Initiative (AHI). Incumbent aims to increase awareness in community about healthrelated issues to improve health decisions through education. The Outreach Health Educator will be providing health education to clients on available program services health maintenance and how barriers of adherence can be addressed to improve clients quality of care in collaboration with the clients care team.

Required field about 75% of the time and take mass transit to travel throughout New York City.

Grantfunded: Contingent upon continuation of grant funding from year to year.

Key Responsibilities to include but not limited to:

  • Develop partnerships at the communitylevel to identify individuals who are underserved uninsured outofcare especially people living with HIV (PLWH) as well as those in need of access to behavioral health and chronic disease management.
  • Conduct outreach activities at various community organizations and healthcare settings to raise awareness on insurance coverage and healthrelated issues providing information on services offered by BSFHC through supporting programs.
  • Educate clients on program services and patient medication management strategies.
  • Engage other communitybased organizations both HIVspecific and nonHIVspecific to develop and maintain an outreach database.
  • Perform outreach to clients who have lost contact or require follow up on medical appointments through various communication methods (i.e. phone calls letters email).
  • Conduct outreach presentations at events such as conferences and healthcare settings throughout the Brooklyn region.
  • Document interventions and follow up client engagement in appropriate system (s) or administrative tracking tools for clients for data collection purposes and submission for accurate reports.
  • Utilize social marketing tools and collaborate with partners to identify and contact hardtoreach populations who are underserved and uninsured.
  • Support the PrEP HIV & HEP C programs by referring partners and community to accessing prevention services particularly among MSM of color.
  • Perform additional duties as requested by the Supervisor.

Education/Experience Required:

  • Bachelors degree required or enrolled in college. Masters degree in healthrelated field such as nursing social work psychology counseling public health or public administration preferred.
  • Experience working in HIV/AIDS LGBTQ and communities of color.
  • Experience delivering community health information to vulnerable populations in Brooklyn and other jurisdictions.

Other Related Skills/Experience:

Successful applicants should be able to describe their previous experience in reaching underserved and hardtoreach populations and connecting individuals to appropriate services. They should have a demonstrated ability to work independently and document all work performed. The suitable candidate should have an understanding of the communitys cultures and ethnicities and knowledge of community assets and resources necessary to effectively respond to needs and gaps in service delivery. Have an ability to prioritize goals and objectives to deliver critical missiondriven services in a timely manner. Have a sensitivity to the needs and situations of a multicultural population with a variety of income levels. Have knowledge of Brooklynbased systems of care. Have the ability to work effectively with a variety of people and work in a team environment.

Annual Salary: $50000
