Long Term Conditions Clinical Coordinator NCL Health Alliance


Us

University College Hospital NHS Trust (UCLH) is looking to employ an experienced personable and organised clinician to lead a small team offering care coordination to people with multiple long term conditions. This is a maternity leave cover post. You will work within an innovative multidisciplinary group and will both initiate and take forward actions to improve the efficiency and effectiveness of patient care.

This is part of an evaluated programme of work the Complex Long Term Conditions Service. The post will be employed by and will sit within UCLH. The programme is being run by the North Central London Health Alliance which carries out joint work on behalf of providers organisations within North Central London. The Health Alliance is hosted by and based within UCLPartners a health innovation partnership. The Health Alliance is recruiting this role on behalf of UCLH.

The Role

The Complex Long Term Conditions Service (CLTCS) has been developed by the NCL Health Alliance together with the ICB as an approach to improve coordination of care for people with complex longterm conditions. We are developing and evaluating a new approach towards the management of multimorbidity.


The LTC clinical coordinator will work with Primary Care Networks (PCNs) and a small team to develop systems and processes to support remote review of High Risk and Complex patients for the PCN building on their experience of working with people who have LTCs.


We are looking for a dynamic clinician or individual with a clinical background to play a critical role as a clinical case coordinator within our friendly and supportive team. Your background might be as a nurse allied health professional physicians associate or mental health practitioner in primary care community services acute care or mental health. You will be familiar with working with people who have longterm conditions.



The Programme
People with long term conditions (LTCs) account for around half of GP appointments two thirds of outpatient appointments and 70% of hospital bed days. Within NCL we anticipate that the population of people living with one or more respiratory or metabolic disease will increase by 8% around 24000 people by 2030.


People with LTCs value careful coordination shared decision making prioritisation and a longerterm perspective. But too often what they find is that services are still characterized by siloed ways of working a focus on acuity and a lack of forward planning.


NCL is making significant progress towards improving efficiency outcomes and quality of care through the Long Term Conditions Locally Commissioned Service (LTC LCS) which aims to embed best practice in the management of LTCs within PCNs.


The NCL Health Alliance is working with the ICB to run an LTC programme that aims to design and test an improved model of care to bring change across the system in the management of LTCs. It aims to complement the LTC LCS through supporting coordination of care across acute community and mental health for the most complex patients.

The LTC Clinical Coordinator is a fulltime fixedterm opportunity as maternity cover to the end of March 2026 (with the possibility of extension) suitable for a Band 8a AHP nurse clinical pharmacist physician associate or manager with clinical/AHP background. The LTC Clinical Coordinator will report to the LTC Consultant.


Remuneration for this post is at NHS Band 8a or equivalent. Secondments would be welcome.
