POSITION SUMMARY:
This is the key administrative position for PostPartum Care Coordination for the department of Obstetrics and Gynecology at Boston Medical Center; the largest safety net hospital in Boston.
The PostPartum Program Coordinator will be responsible for improving our patients access to and uptake of postpartum care. The postpartum program coordinator will assist in the design
of performance objectives and implement and monitor programming to meet desired outcomes. Along with the medical director operations managers and prenatal team; the program
coordinator will plan to improve health outcomes by increasing engagement with postpartum care and helping mothers transition into primary care.

Schedule: Full Time
ESSENTIAL RESPONSIBILITIES / DUTIES:
Ensure connection and follow through to primary care in the first year postpartum for women with high risk comorbidities:
Hypertension
Diabetes type 1 or 2
Women w severe preterm preeclampsia
Must adhere to all of BMCs RESPECT behavioral standards.

JOB REQUIREMENTS
EDUCATION:
Bachelors Degree (or equivalent experience) required preferably in healthcare or related field.
EXPERIENCE:
Minimum of 4 years relevant experience required. Nonprofit program coordination is a plus. Knowledge of program evaluation is helpful.
