Job Summary As part of the Choosing Childbirth grant the Family Services Navigator assists expectant and new parents with navigating community resources and accessing services to prenatal medical and mental health care insurance enrollment social supports basic life essentials housing and transportation. Collaborates with Parent Educators and other staff to ensure effective resource connection. Facilitates human capital development and training opportunities. Supports the design implementation and evaluation of the Choosing Childbirth grant pilot through supporting the development of participant recruitment strategies enrollment and facilitation of warm referrals to community partner organizations. Facilitates warm and welcoming environments to serve diverse participant populations in a nonjudgmental way. Builds mutual trust and respect with participants which supports collaborative partnerships and facilitates experiences linked to learning. Ensures timely data entry into all appropriate referral tracking systems with high degree of accuracy and accountability. Provides subject matter consultations for Family Advancement team. Sets precedent within limits and/or administers operating rules and procedures under management guidance. Performs intermediate troubleshooting or analysis; problems require interpretation of data using generally prescribed procedures. Frequent interaction to provide basic information and/or occasional interaction requiring detailed procedural explanations or technical responses.
Responsibilities/Essential Functions - Partner with internal and external partners to provide warm referrals to basic needs and emergency service community agencies for families enrolled with Family Advancement with a nonjudgmental approach. Cultivate and maintain collaborative partnerships with key community partners and connect families to community services and events through curated and targeted outreach.
- In coordination with staff designs and facilitates experiences linked to learning (to simulate growth in cognitive language social and motor skills) for current families and children as well as prenatal moms and their partners.
- Assess family needs make appropriate referrals and provide followup to ensure access of services.
- Check and document all parental consent forms prior to administering any health screenings and assessments; obtain releases for medical information and contact agencies clinics and health care providers to obtain pertinent medical/dental information.
- In coordination with Admission and Recruitment develop and implement recruitment plans to support the enrollment of and Choosing Childbirth via varied communication and recruitment methods (i.e. phone text email and provide translation services to families as needed)
- In coordination with health and behavioral health staff administer physical and mental health screenings assists with screenings and examinations necessary followup and supports communication of results to parents including assisting parents with navigating any followup referrals resulting from screenings.
- Provide reporting support to leadership about family needs referrals and community partnerships. Provide input on recommendations grant updates and strategic partnerships.
- Enter referrals and followup documentation into all appropriate systems and support the documentation of referrals made by others on the team.
- Support the Choosing Childbirth planning implementation monitoring and evaluation phases by assisting with human centered design efforts gathering feedback from participants through focus groups survey or other input mechanisms.
- Performs other duties as assigned or requested.
Required Education/Experience - Bachelors Degree Required Bachelors Degree in Early Childhood Education Child Development Health Human Services or other relevant social sciences field preferred. Will consider eight years of experience in lieu of education or a combination of education and experience.
AND - 3 years of experience working with families and helping them navigate community resources and facilitating group experiences linked to learning.
- Spanish and English fluency required.
Certification/license
- Must obtain food handlers permit and CPR/First Aid Certification within 30 days of employment.
- Must obtain state certification for vision screening within one year of employment.
Knowledge/Skills/Abilities - Must have proven successful communication and interpersonal relationship skills (empathy conscientious ethical nonjudgmental) to maintain effective and satisfactory working relationships with intradepartment and interdepartment staff vendors clients visitors and contractors.
- Expertise engaging with diverse families and a proven ability to build trusting relationships
- Knowledge of community resources and health providers and community engagement experience
- Ability to follow oral and written instructions with ability to read interpret and present information in English efficiently and effectively.
- Must be able to accurately audit records and ensure compliance with written or oral guidelines.

- Must maintain strict confidentiality of extremely sensitive data records files conversations etc.

Work location Inperson work and collaboration are requirements for this position. Employees are expected to physically report to assigned work site as scheduled by supervisor. At Agency discretion this position may be allowed to engage in remote work for specified periods of time or a determined percentage of work schedule; however this is not guaranteed. Temporary remote workdays may be approved for circumstances such as but not limited to inclement weather special projects or travel. These arrangements are approved on an asneeded basis with no expectation of ongoing continuance. All remote work arrangements are made on a casebycase basis focusing first on the business needs of the organization.
Essential Physical & Mental Requirements Work is a normal office environment. Employees are subject to standing stooping walking lifting and carrying objects throughout their daily job effort involving lifting amounts between 5 and 25 pounds required. Work requires moderate attention to detail and deadlines between 15% and 45% of the time. Driving is required intermittently with exposure to normal traffic hazards. Pay Band Grade: 13
