POSITION SUMMARY:
We are seeking an experienced Nurse Practitioner who is energetic and passionate about helping and working with a diverse population. The Family Nurse Practitioner will evaluate diagnose and treat pediatric adult medicine and gynecology patients.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Involved with direct patient physical assessment education and health planning.
- Assists in the coordination of services with SJH Case Managers other disciplines and patients insurance plans
- Clinical core privileges will include the management treatment and ordering performing and interpreting labs radiography and other diagnostics test for acute critical care disorders in the adult critical care population
- Advocates for patient in arrangement of medically necessary services;
- Communicates with communitys resources to ascertain adequate assistance for patients in need;
- Refers patients to specialty services when necessary;
- Consults with collaborating physician and other members of the health care team to establish plan of care;
- Maintains continuity of care by regular patient followup;
- Participates in quality assurance process to assure optimal care delivery;
- Provides comprehensive case management services that include emphasis on health promotion and disease prevention;
- Monitors patient outcome in accordance to the practice protocols;
- Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning;
- Acts as a resource person for the nursing and medical assistant staff;
- Conducts education in service on practice issues;
- Other duties as assigned

QUALIFICATIONS:
Minimum 1 year experience as NP
Graduated from approved Nurse Practitioner Program;
Licensed as Nurse Practitioner in New Jersey
Basic CPR (BCLS) certification required;
CDS required;
DEA Certification required
