Job Description:
To provide patientfocused pharmacy services including monitoring of therapy interventions basic pharmacokinetics education and medication distribution. To support the mission activities and programs of the division department and the Hospital.
Key Responsibilities:
Manage medication therapy for transplant patients
Collaborate closely with physicians nurses and other healthcare professionals; notify the appropriate physician or healthcare professional of any issues regarding compliance or medication therapy regimens
Actively monitor patientspecific response to prescribe therapies reviewing potential adverse drug reactions and drug interactions to optimize therapy. Reevaluate prescribed therapy and makes recommendations for changes when appropriate
Provide patient education regarding transplant medications
Accurately document patients medication history profile and evaluations
The position will be 10hour day shifts
Bachelors degree in Pharmacy required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Licensed Pharmacist by the Alabama Board of Pharmacy (ALBOP) required.
