Join a dynamic multidisciplinary HIV clinical care program which is funded by the national Ryan White HIV/AIDS program.
We are seeking a Social Worker to:
- Utilize both clinical and community based interventions to assist a diverse patient population facing complex medical and psychosocial needs.
- Anticipate challenges and barriers that make it difficult for patients to stay engaged in HIV care and help improve selfmanagement of their illness within a medical case management framework.
- Support adult and pediatric patient populations.

Qualifications:
- Masters Degree in Social Work Required
Work Experience:
- 2 years prior hospital based social work experience Preferred
- 2 years prior hospital or communitybased social work experience if supporting the HIV Clinic Preferred
- 2 years prior hospital or clinic based pediatric experience if supporting American Family Childrens Hospital (AFCH) Preferred
Licenses & Certifications:
- Certified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin within 90 Days Required or
- Certified Independent Social Worker (CISW) in the state of Wisconsin within 90 Days Required or
- Licensed Clinical Social Worker (LCSW) in the state of Wisconsin within 90 Days Required
