Summary
This position will collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patientcentered plans of care for both individual patients and the medical community.

Essential Functions
Provides psychosocial assessments of patients and families with social emotional interpersonal and/or environmental issues. Formulates biopsychosocial assessment disposition and treatment plans.
Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.

May need assistance in promoting collaboration among healthcare team members other colleagues and the organization to support and enhance patient care.
