As an Endocrinology Community Health Worker you will work closely with the diabetes team and patients to ensure they receive the resources and support they need to manage their condition effectively.
This flexible hybrid position offers daytime hours (approximately 10 hours per week) and allows you to split time between inperson and remote work. Ability to go predominately remote after successful onsite training is complete.
Job Summary
What Youll Do:Empower Patients Help individuals with diabetes access necessary resources such as medication assistance insurance support educational resources and more.
Strengthen Community Connections Connect patients with senior centers local support groups financial assistance programs and food security resources.
Collaborate with Healthcare Teams Work alongside the endocrinology and iCMP care teams to develop a seamless bridge of care for patients.
Qualifications
Qualifications:
- High School Diploma/GED required; Associate or Bachelors degree preferred.
- Prior experience in healthcare social work or a related field preferred.
- Valid drivers license and ability to travel within the community as needed.

- Knowledge of community resources and social services that support individuals with diabetes.
- Bilingual skills preferred (Spanish or Haitian Creole preferred) to better serve our diverse patient population.
- Excellent communication problemsolving and organizational skills to manage a caseload of patients effectively.
