As a Palliative Case Manager you will be joining a collaborative team that is dedicated to helping others in need and supporting one another. As a Palliative Case Manager you will develop implement and review healthcare plans for patients enrolled in our Palliative Care program that are chronically ill helping them understand and choose the best healthcare options for them. Our Palliative team is committed to providing simply the best care.

Manage your own schedule work 40% from home/telephonic and 60% in patients home.
Limited on call 1 week every 68 weeks (telephone calls only)

Responsibilities:
Assist in providing relief from symptoms and stress of a chronic illness while working together and consulting with a patients other doctors to provide an extra layer of support in the home.
Assess community resource needs to manage patients comfort health and safety.
Collaborate with Homecare and Hospice to coordinate care.
Weekly Palliative contact with at least one monthly visit in person.
Clinical and critical thinking skills to provide the best care and reduce hospitalizations.
Be competent in knowledge about different levels of care and when its appropriate to offer them.
Foster a culture of teamwork respect and collaboration among the staff and interdisciplinary team members.
Reliable transportation and ability to visit patients in their homes.
Qualifications:
Associate degree w/RN license
Nursing: 1 year (preferred)
Case management: 1 year (preferred)
