RBHAhas an exciting new opportunity for a Case Manager to join our team of Life Changers! The selected individual will performintermediate professional work assessing client needs developing implementing and monitoring service delivery and assistance plans coordinating and monitoring services with other agencies counseling and assisting clients maintaining records and files preparing reports and related work as apparent or assigned in the Crisis Stabilization Unit (CSU). Work is performed under the moderate supervision of the assigned Clinical Supervisor.
Essential Functions
Provide case management and discharge planning services to clients in CSU including engagement and rapportbuilding referring and connecting with appropriate services including longterm mental health substance abuse medical and natural supports; referrals for housing monitoring the agreed upon support plan and transitioning of services to community supports for continuity of care.
Collaborates and coordinates with Region 4 CSBs including Housing Specialists and Peers on a frequent basis.
Assists clients in applying for Medicaid; Care Coordination with Managed Care Organizations (MCOs).
Assesses client service needs capabilities and appropriateness for services; presents options and services based upon a needs assessment.
Makes referrals and linkages to appropriate agencies for services coordinates client services and treatment with multiple service providers and agencies.
Evaluates the quality of services provided and changes in clients condition and supports clients as necessary; evaluates clients environment for safety security negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.
Serves as liaison to public agencies and provides information regarding RBH programs and services.
Attends interdisciplinary team meetings to discuss decisions for client plan of care; arranges discharge transportation.
Provides crisis intervention and prevention services.
Prepares a variety of reports; prepares and maintains client charts and other records.
Attends meetings staffings and conferences as they relate to client staff and program needs.
Assists in curriculum development and resource tools.
