Position: Home Health LPN

Job Summary:
We are currently seeking a highly skilled and compassionate Home Health LPN to join our team in Milford PA. The ideal candidate will have a strong background in home health care and a passion for helping patients maintain their independence and quality of life.
Responsibilities:
Provide skilled nursing care to patients in their homes including medication management wound care and other nursing procedures as needed
Conduct initial assessments and develop individualized care plans for each patient
Collaborate with other healthcare professionals to ensure the best possible outcomes for patients
Educate patients and their families on disease management and selfcare techniques
Monitor and report changes in patients conditions to the appropriate healthcare team members
Maintain accurate and uptodate patient records
Adhere to all state and federal regulations as well as company policies and procedures
Qualifications:
Current LPN license in the state of Pennsylvania
Minimum of 1 year of experience in home health care
Strong clinical skills and knowledge of nursing procedures
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation and willingness to travel to patients homes
CPR certification
Ability to pass a background check and drug screening
