We are a comprehensive nonprofit organization that serves older adults and people with disabilities in King County. Our mission is to partner with older adults to provide accessible and inclusive services so they can age their way.
We are currently seeking a Case Managerto join our assistance services team in Seattle WA.
What will YOU do
The Case Manager provides comprehensive case management that includes screening and assessing clients; creating implementing and monitoring individual service plans; crisis intervention; connecting clients to health mental health and other community resources. This is a new program in collaboration with HealthierHere and the team will take part in the first Community Hub cohort. Case Managers will receive mandatory trainings through HealthierHere. Some inperson work will be required.
Pay Rate:$31.82 per hour.
Work Schedule:Monday Through Friday 40 hours per week.

Responsibilities:
- Provide comprehensive case management that includes screening and assessing clients; creating implementing and monitoring individual service plans; crisis intervention; connecting clients to health mental health and other community resources.
- Conduct screening assessment intake and discharge in accordance with program and contract policies and procedures and utilizing program tools.
- Generate update and document clientcentered care plans based on the clients goals and in accordance with the program and contract policies and procedures for client eligibility.
- Assist clients in the identification of barriers to accessing care and to making behavior changes which positively impact their health or the health of others.
- Conduct home community and/or virtual visits as necessary.
- Prepare case file data documentation and/or reports. Develop and maintain case records which document decisions and provide record of case plans.
- Act as liaison between vulnerable populations and the health care system/needed services while offering support and informal counseling to ensure responsive tailored and accessible care.
- Coordinate services with other agencies. Refer and monitor the progress of referrals to other agencies.
- Explain program policies to individuals families and members of the community.
Requirements:
- Bachelors degree or equivalent.
- Three (3) years of paid or volunteer experience providing services to aging individuals.
- To better enable Sound Generations to meet the needs of its clients and program participants bilingual (Spanish and English) candidates are preferred.
