Job Description
Facility Location:
Franciscan Neurology Associates Tacoma
Setting: Clinic
Duties: New patients follow up visits therapeutic treatments (Botox ONB. etc.)
FTE: 1.0
Days per month: 5 days/week
Required Procedures: Minimal Botox ONB etc.
Patient Volume: 1315 per day
EMR: EPIC
Support Staff: Established LPN or MA already in clinic
Reason for Coverage: Hospitalist model changing excluding ambulatory visits

Timeframe: 4590 days
Requirements: MD/DO General Neurologist
Credentialing with other facilities automatically credentialed at all but St. Michaels
Billing enrollment Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less No
