About the role
The role provides a palliative care consultancy service to patients living in 22 local residential aged care facilities.
The role will analyse the educational needs of RACF workforce and provide specialist
Needs Rounding in 22 RACFs to improve palliative care skills of nursing and care workers in facilities.
Working within a multidisciplinary team to enhance the links with internal/external stakeholders to improve end of life care for patients living in RACFs.
About you
Current registration as a Registered Nurse (Division 1) and endorsed by the Nursing and Midwifery Board as a Nurse Practitioner specialising in Acute Aged Care and or Palliative Care or working towards same as per NMBA guidelines on endorsement as a Nurse Practitioner
Proven high level knowledge and expertise in complex client centered palliative care consultancy practice development activities including mentoring education policy development and quality improvement and current unrestricted NSW Class C license.
Demonstrate ability to work collaboratively with external agencies eg: Residential Aged Care Sector Primary Health Network to support the strategic and operational requirements of the hospital.
Demonstrated high level palliative care knowledge and advanced clinical nursing skills including comprehensive assessment interpretation of diagnostics implementation of treatment modalities and relevant referral skills applied to a broad range of challenging complex and changing situations.
Demonstrated ability to work autonomously and within the interdisciplinary team and multiservice environment and demonstrated commitment to ongoing professional development and quality improvement projects.
