Physical Therapist Position Overview:
As a Physical Therapist at Pinnacle Home Care youll play a key role in delivering personalized outcomedriven care to patients in their homes. Working in close collaboration with a multidisciplinary team and under the direction of a physician youll empower patients to restore mobility independence and quality of life.
Key Responsibilities:
Conduct initial and ongoing patient assessments including OASIS documentation
Develop and implement individualized treatment plans based on physician orders
Provide therapy interventions to improve mobility strength endurance and overall function
Educate patients and caregivers on home exercise programs and safety strategies
Collaborate with nurses physicians and care team members to ensure coordinated care
Document patient progress changes and outcomes clearly and promptly
Supervise Physical Therapist Assistants (PTAs) every 30 days or as required
Participate in care plan updates and ensure alignment with clinical standards
Qualifications:
Graduate of an accredited Physical Therapy program
Licensed Physical Therapist in the state of Florida
Minimum of two years of clinical experience in physical therapy (home health experience preferred)
Current BLS Certification
Valid drivers license and access to a reliable insured vehicle
Strong communication observational and critical thinking skills
Ability to work independently with selfmotivation and excellent time management
