Interim HealthCare is hiring!
Were looking for a detailoriented techsavvy and customer servicedriven Intake Coordinator to join our highperforming team.
This is a fulltime highimpact position that supports our Home Health Hospice and Personalized Care at Home services. Youll be the friendly and efficient first point of contact for referralscoordinating directly with physicians hospital ERs sales team members case managers clients and families to ensure fast accurate intake and smooth transitions of care.
Key Responsibilities Include:
- Managing incoming referrals with speed accuracy and professionalism
- Working closely with inside sales ERs and physicians for timely patient disposition
- Tracking and reporting on daily KPIs tied to referral activity and conversion
- Coordinating with families case managers and internal teams to launch care quickly
- Utilizing systems such as CarePort ECIN Legacy Allscripts and Adobe Acrobat
- Maintaining a hospital referral list and ensuring followup and compliance
What Were Looking For:
- Experience in healthcare intake inside sales or referral management
- Comfort with EMRs digital tools and multitasking in a fastpaced environment
- Strong communication and organizational skills
- Previous Home Health Hospice or Private Duty experience a plus
- Knowledge of payer guidelines and HIPAA compliance
