Job Description

Care Manager

Location: Springfield MO

Department/Program: Maternal and Family Programs

Primary Shift: MF 7:00 am 3:30 pm (some evening hours as needed)

Job Classification: FullTime

FLSA Classification: NonExempt

Salary: $18.00 $19.00 per hour

Travel Required (Y/N %):Y 50% local

LifeHouse is a residential program that provides a safe nurturing home for pregnant women experiencing homelessness and youll be at the heart of helping residents build strong hopefilled futures. Youll also serve families through our Healthy Moms Healthy Babies program a communitybased initiative offering pregnancy and parenting education to support healthy pregnancies confident parenting and longterm stability. What Youll Do:

  • Provide comprehensive case management to residents and aftercare clients for the purpose of selfsufficiency.
  • Attending community networking meetings by representing the agency.
  • Create and update individualized care plans for residents and clients per the requirements and needs of the individual.
  • Assist residents with transportation.
  • Conduct intakes facetoface interviews and orient new residents.
  • Provide case management with each client in combination with phone and facetoface in varied settings including the clients home.
  • Ensure residents complete chores pass room checks and uphold their commitment to the agreement and expectations of the program. This includes but is not limited to coordinating and monitoring class schedules utilizing corrective action plans administering drug screenings etc.
  • Complete written and databased documentation (i.e. internal documentation case notes housing plan etc)
  • Advocate and interact with other service providers parent educators housing agencies churches and community organizations on behalf of participants as well as facilitate referrals.
  • Perform other related duties as assigned.

Possesses a strong commitment to the mission of Catholic Charities including the understanding of Catholic Social Teaching and the ability to demonstrate the philosophy and values of the agency. The identity of the agency is clearly Catholic and as such Catholic Charities adheres to the social and moral teachings of the Catholic Church.

All you need for success:

Minimum Qualifications

Required:

  • Bachelors degree in social work (BSW) or equivalent human service degree.
  • This position requires a valid drivers license a reliable vehicle and proof of insurance.

Preferred:

  • Experience in case management and homelessness/atrisk of homelessness populations.
  • Knowledge of social services and referral sources for families.
  • Motivational interviewing skills.
