Are you looking to make a difference Jefferson Healthcare is seeking a fulltime Licensed Independent Clinical Social Worker to join our Primary Care Team. As a member of the Integrated Behavioral Health team you will provide shortterm services to assist patients of all ages and families in coping with personal financial psychosocial and environmental problems associated with the patients medical and/or psychiatric diagnosis. Our Primary Care group is a collaborative group that is committed to our patients and community. Schedule: 1.0 FTE; 40 hours/week; Day Shift; No weekends; Schedule flexibility

- Washington State Licensed Clinical Independent Social Worker.
- Two (2) years of recent LCISW experience in a health care setting.
- Minimum two years of Social Worker experience in a healthcare setting clinical social work in a similar setting or case management role preferred. Medical social work experience preferred.
- Experience in working with specific patient populations that may include but are not limited to children frail elderly vulnerable adults and patients impacted by mental illness.
- Previous experience in the AIMS collaborative care primary care clinic is a plus.
- May require a valid Washington state drivers license.
