Job Description

Onboarding Support Professional (OSP)

Nature and scope of Job:

The Onboarding Support Professional (OSP) will work with our client after completion of Intake. They will support the individual by completing their ISP goals and any support needed for ADLs/IADLs. The OSS will assist the pairing team with finding a qualified DSP by providing them with additional information regarding the clients support needs. They will help introduce the family to DSPC by getting them acquainted with our office locations and systems. The OSS may assist with training the identified longterm DSP(s).

Salary:$24 Hourly

Location to travel to: Marion Polk and Linn County.

Qualifications:

  • Be at least 18 years of age
  • Pass a Background Check
  • Minimum 1 year experience of working with the I/DD Community
  • CPR/First Aide Training within 30 days of hire (will provide class opportunities)
  • Valid Drivers License
  • Reliable transportation
  • Smart Phone
  • OIS Training Certification (Will provide class opportunities)
  • Ability to follow a PBSP
  • Willing to transport Client in their own vehicle
  • Flexibility

Job functions and Responsibilities:

  • Provide ongoing DSP support to the client while the Pairing team look for a longterm DSP.
    • Assist the individual with ADLs/IADLs
    • Support the individual with progressing in completing their ISP goals.
    • Assist with minimal house chores
    • Complete shift notes at the end of each scheduled shift.
    • Complete any other required documentation for each individual (Sleep tracking MAR Behavior tracking etc.)
    • Maintain health and safety of the individual you are supporting
    • Communicate with DSPC and assigned family
    • Follow employee handbook and other DSPC Policies.
  • Provide the family the DSPC in the home binder
  • Assist the family and/or client by filling out the DSPC in the home binder and include the necessary ISP Documents.
  • Take the individual on a tour to our main office location
  • Provide the client with the opportunity to go to Connections corner
  • Provide the opportunity to explore upcoming DSPC events
  • Assisting the family with communicating DSP needs to our pairing team to help find the best DSP match
  • Be available to assist with training/handoff to identified longtermDSP
