Full Time 1.0 FTE

Monday Friday 8:00 AM 4:30 PM

Responsibilities

  • Provide personalized oneonone physical therapy care to patients in their homes focusing on helping them regain independence and improve their quality of life.
  • Collaborate with patients families and the care team to develop and implement individualized care plans that align with patient goals.
  • Participate in a nationally recognized healthcare team by ensuring highquality physical therapy care and contributing to a supportive patientfocused environment.

Qualifications

  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE) a successor organization of CAPTE or an education program outside the United States determined to be substantially equivalent to physical therapist entrylevel education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
  • Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.
  • Licensed as a PT by the state in which practicing unless licensure does not apply within state regulation
  • One year experience caring for similar patient population as to be assigned.
