A. Responsibility
- Screen and schedule applicants for eligibility
- Follow up on referrals by contacting the client and completing the PreScreen Eligibility Checklist and assessing client needs.
- Arrange program intake appointment for clients that meet program eligibility standards
- Case manager should assist the client in completing Determining Household Income form 4.8.6 to assess monetary income within the household.
- Provide resources as applicable to assist the client in obtaining increased income reduce expenses etc. Update client progress on these measures each month in which the client applies for assistance using Ongoing Followup Plan form 4.8.12
- If client meets program eligibility standards Case Manager should arrange a client housing search appointment unless clients current residence is under FMR and the landlord will accept housing/residence HOPWA requirements.
- Case manager should review all documentation brought in by the client to ensure it meets the requirements of HUD / COH.
- Check the property ownership to ensure the information provided on the landlords W9 matches information on file with HCAD
- Contact client landlord to schedule the onsite unit inspection
- When applicable case manager must send Finance Specialist an internal email entitled TBRA Request
B. Education Training Skills Knowledge and Abilities
- Associates degree required
- 23 year experience preferred
- Ability to read write and communicate effectively. Knowledge and skill as a case manager working in a social service/human service environment
- Ability to mentally and physically cope with stresses cause by regular exposure to the clients behavioral health issues and respond appropriately to client crises and seek help in coping when needed through supervision case consolation or counseling when needed.
- Ability to respond compassionately to persons who are HIV positive and their partners/family
- Ability to develop creative responses to complicated problems involving indigent clients
- A perspective accepting of a wide variety of peoples problems attitudes and lifestyles. Good interpersonal skills and the ability to work on a team.
