Job Description

Position Summary:

Chemik Hospice Care LLC is seeking a compassionate nonjudgmental and spiritually grounded Hospice Chaplain to provide emotional and spiritual support to patients and families facing terminal illness. As a vital member of the interdisciplinary team (IDT) the Chaplain helps ensure holistic care that honors the beliefs values and traditions of each individual regardless of faith or background.

Key Responsibilities:

  • Conduct initial spiritual assessments within five 5 days of admission and as needed.
  • Provide ongoing spiritual support and counseling to patients and families based on their individual needs and preferences.
  • Offer nondenominational support respecting all belief systems and cultural backgrounds.
  • Assist patients and families in addressing issues of meaning grief forgiveness hope and legacy.
  • Participate in interdisciplinary group (IDG) meetings to contribute to the development and revision of individualized care plans.
  • Provide or coordinate spiritual rituals prayer or sacraments when requested by patients or families.
  • Offer bereavement support as part of the hospice bereavement program.
  • Coordinate with community clergy or spiritual advisors as needed.
  • Maintain timely and accurate documentation of all spiritual care services in the electronic health record (EHR).
  • Provide support to hospice staff as needed to address emotional or spiritual stress related to caregiving.

Qualifications:

  • Masters degree in Divinity Theology Pastoral Counseling or related field preferred.
  • Ordination or endorsement by a recognized faith group or religious organization.
  • Experience in hospice palliative care or healthcare setting preferred.
  • Strong interpersonal listening and communication skills.
  • Ability to provide interfaith and culturally sensitive spiritual care.
  • Valid drivers license and reliable transportation for patient visits.
