We have an exciting opportunity for a seasoned Case Manager. For us the right candidate will have a have made a career commitment to seniors and have a true passion for their wellbeing.

What we need you to have:
- Ability to perform resident assessments
- Create and maintain individualized service plans
- Full knowledge ofDOH regulations
- Scheduling of resident appointments including but not limited to transportation
- Assure compliance with accordance to the state regulations
- Schedule and conduct family meetings
- Computer literate
