Job Description
FieldBased Nurse (RN or LPN)
Schedule: MondayFriday 8:00 AM4:00 PM (some flexibility required)
OnCall: Shared rotation 24/7 coverage consultbased with occasional response
Location: Hammond LA
Pay:
LPN: $27.50/hour
RN: $38.75/hour

If youre a nurse whos tired of 12hour shifts bedside burnout or being a number on a staffing board this is your sign. OPTIONS is hiring a licensed RN or LPN to support adults with developmental disabilities in a community setting not a hospital not a clinic and not home health.
This role is independent fieldbased relationshipcentered and deeply rooted in OPTIONS mission of helping people live with dignity purpose and real connection. Youll support a small number of residents in local group homes no facility chaos no conveyorbelt care.
What Youll Do
Conduct routine health assessments and document medical needs
Monitor for illness med changes or health concerns
Attend and support participants at medical appointments
Act as a liaison between doctors pharmacies families and support teams
Audit medication logs and eMARS to ensure regulatory compliance
Provide nursing procedures as needed (per license level)
Rotate oncall responsibilities with team nurses phonebased consultfirst approach

Youre a Fit If You
Hold a current Louisiana LPN or RN license
Have 25 years of nursing experience
Own a vehicle and have a clean driving record
Work well independently and communicate with clarity
Are organized flexible and techcomfortable (we document electronically)
