Homestead Village is a thriving nonprofit CARFCCAC accredited Life Plan Community located just outside vibrant Downtown Lancaster. We are seeking a highly skilled enthusiastic & passionate Licensed Practical Nurse (LPN) to join our compassionate community as we enhance independence and dignity for all our Residents.
Position/Schedule: Part Time nonexempt; 6:45PM 7:15AM; 2 shifts per week rotating 3 week schedule
- Week 1: Wednesday & Thursday
- Week 2: Monday & Tuesday
- Week 3: Friday & Saturday
Location:
- 26 Bed Memory Support Households: Gelhard House & Harvest House
- Main Apartment Building; Supportive Servies
Base Pay: Determined based upon years licensed and experience. LPNs with less than 1 year experience start at $25.00; Plus applicable shift & weekend differentials
Shift/Weekend Differentials:
- $1.50 additional from 3:00pm11:00pm (MonFri)
- $0.75 additional from 11:00pm7:00am (MonFri)
- $1.25 additional on weekends (Saturday & Sunday)
Qualifications:
- Currently licensed in the Commonwealth of PA as a Licensed Practical Nurse
- Minimum of three years as an LPN
- Minimum of three years of supervisory experience
- Experienced in developing support plans for elders
- Familiarity with Personal Care Home Regulations
- Handson experience in ADL assistance
- CPR/AED certified or able to be certified
- Written & verbal skills to communicate with client families and staff.
- Able to perform the essential job functions of the job description.
Responsibilities (the following is an overview of the responsibilities of the position and should not be considered all inclusive):
- Establish respectful warm relationship with residents perform rounds
- Admission of residents
- Develop/update of resident prescreen assessment and support plans
- Communicate as necessary with physicians and resident families.
- Staff supervision and coaching
- Assist with meeting resident activities of daily living as needed/indicated.
- Assist with medication administration as needed
- Participate in weekly PCH Supervisory meetings
- Assist with emergency response campuswide as needed
- Attend all required staff meetings and complete monthly education on Relias by dates due.
- Adhere to all agency policies and procedures federal state and local regulations and OSHA safety standards.
- Perform other duties as assigned
