What Youll Do in This Role:

The Physical Therapist evaluates and assesses participants to determine their mobility level and functional abilities. They provide and recommend therapeutic exercises gait training ambulation strategies and other physical therapy interventions. While working under the direct or indirect supervision of the Center Manager or designee the Physical Therapist operates independently in applying advanced knowledge and skills. This role also includes planning and leading group exercise programs for maintenance therapy. The Physical Therapist is responsible for overseeing the overall physical therapy program at Thome PACE.

Primary Responsibilities:

  • Conducts initial and periodic reassessments (at least every six months) of participants physical condition and functional status; assessments must be completed prior to the scheduled Interdisciplinary Team (IDT) meeting. Communicates any changes in participant condition to the team.
  • Develops and updates individualized Plans of Care establishes treatment programs when indicated sets goals and provides therapeutic interventions.
  • Documents all treatments in the participants chart including signing and dating progress notes.
  • Completes assessments and discharge summaries; plans and develops maintenance exercise and therapy programs tailored to participants needs.
  • Actively participates as a member of the Interdisciplinary Team including attendance at case conferences and staff meetings.
  • Coordinates 24hour care delivery and provides training to family members caregivers and Thome PACE staff on implementing relevant components of the treatment plan.
  • Supervises program aides and other staff involved in carrying out the physical therapy treatment plan.
  • Assesses needs for adaptive and durable medical equipment specialized footwear and prosthetics making appropriate recommendations to the IDT.
  • Contributes to discharge planning for participants transitioning from hospitals assisted living or skilled nursing facilities.

What Youll Need:

  • Bachelors degree in physical therapy (if obtained prior to the year 2000; masters degree in physical therapy required if obtained after 2000.
  • Current CPR certification or willingness to obtain upon hire.
  • Demonstrated clinical competency and experience working with the geriatric population.
